Before mailing your donation, please print off and include the Medical Resource Donation form, click here.  You can simply mail your donation and the form without prior approval to our warehouse at:

We will send you a Letter of Receipt once we process your donated resource(s).  Tax deductions are available for your charitable contribution. Simply present the Letter of Receipt to the IRS with your tax filing.
